Office Policies
We are open to serve you, answer questions or schedule an appointment during the following hours:
Monday through Thursday: 8 a.m. - 4 p.m.
Appointments
A scheduled appointment time has been reserved specially for you. We request 24 hours notice if you need to cancel or reschedule your appointment.
Payment Options
Payment in full is expected the day the services are rendered. Financial options include check, Mastercard , Discover or Visa.
We also offer an affordable new option with 0% interest up to 24 months through ChaseHealthAdvance or Care Credit. There is no application fee and no prepayment penalty. This can be used for your total dental fees for our office and if you choose, you can also include your restorative dental fees. The application process is quick and easy.
Insurance
Our office is committed to helping you maximize your insurance benefits. We will help you file your claim, but the patient is responsible for the total payment. We do not file for Medicare, as it does not pay for any dental work.
New Patient Letter
New patients are mailed a letter containing information about Dr. Feldman, office policies, a map and some forms to be completed prior to their initial visit.
