First Visit
In order to stream lined your first visit, please bring your completed forms, (if they are not completed, come ten minutes early to do so). The forms are on the website or were mailed to you in your new patient letter. The forms are your personal health history and notice of privacy. Please also bring any X-rays, cat scans or dental insurance information, if applicable.
Your initial exam will last approximately one hour. Our nurse or surgical assistant will review your health history. Any necessary x-rays and /or cat scans will be taken at this time. We have, on cite, our own Kodak cat scan machine and the information it provides allows Dr. Feldman to diagnosis with even greater accuracy as it shows areas of the jaw, teeth and bone that were unable to be seen with conventional dental X-rays. A lot of the initial exam done by Dr. Feldman can now be done outside your mouth utilizing our new technology. He will use the XCPT software which he developed to document all the findings. You will be watching, asking questions and participating during the consult. We will print and /or e-mail the treatment plan for you so when you leave our office you will have a visual record of your consultation along with the fees. Dr. Feldman will e-mail your treatment plan to your general dentist and discus all your pertinent issues. If you do not have a general dentist, we will help you in the selection of one.
Our primary concern is your optimum dental health. We encourage all patients to discuss any or all of their concerns. We are here to be of service to you. An informed patient is better able to make a comfortable decision.
